Position Overview:

We are seeking a skilled and compassionate Community Nurse to join our dynamic team. The Community Nurse will play a vital role in providing comprehensive nursing care, health education, and support services to individuals and families within our community. Responsibilities:

• Provide skilled nursing care to patients in their homes, including assessments, wound care, medication management, and symptom monitoring.
• Develop individualized care plans in collaboration with patients, families, and healthcare team members to address their unique needs and goals.
• Conduct health assessments and screenings, provide health education, and promote preventive care practices within the community.
• Coordinate and collaborate with interdisciplinary team members, including physicians, therapists, social workers, and community agencies, to ensure continuity of care.
• Advocate for patients' rights and access to healthcare services, addressing social determinants of health and barriers to care.
• Maintain accurate and timely documentation of patient assessments, care plans, and interventions in accordance with regulatory standards and organizational policies.
• Participate in quality improvement initiatives, research projects, and continuing education activities to enhance nursing practice and patient outcomes.

Qualifications:

• Registered Nurse (RN) license with Valid APHRA
• Bachelor of Science in Nursing (BSN) preferred.
• Nursing experience, preferably in community or home health settings.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, interpersonal, and teamwork abilities.
• Knowledge of community resources, healthcare delivery systems, and population health principles.
• Ability to work independently, prioritise tasks, and adapt to changing patient needs and environments.
• CPR certification and other relevant certifications may be required.
• Driver’s preferred
